Before obtaining or renewing an ADR certificate, you must submit an application form to the CBR/CCV for the new ADR pass.
You can download the original form from the CBR website and fill it out digitally. Save the form as a document on your computer first, otherwise a watermark may remain visible.Is this not possible or do you prefer to fill out the form manually? We will also send you the form with the invitation to the course.
On this form, among other things, you must place your signature and paste your passport photo. The back of the passport photo must have your name and date of birth written on it (in case it becomes detached from the form). It is important that your signature on the application form is placed within the lines.
You can submit the form to us only on the first day of the course.
